Eterna Life & Health is not affiliated with or endorsed by the U.S. government, Medicare, the Centers for Medicare & Medicaid Services, HealthCare.gov, or the Health Insurance Marketplace.

The information provided on this Website is for general informational purposes and is not intended to constitute legal, tax, financial, or medical advice.
Insurance laws, regulations, eligibility requirements, premiums, benefits, provider networks, formularies, cost-sharing amounts, subsidies, and plan availability may change and may vary based on location and individual circumstances.

4. Medicare Plan Information
Eterna Life & Health does not offer every Medicare plan available in every service area. The information we provide is limited to plans we are authorized and contracted to offer.
Current Medicare plan availability depends on the beneficiary’s ZIP code, county, eligibility, enrollment period, carrier contracts, and other applicable requirements.
Consumers may contact Medicare.gov, call 1-800-MEDICARE, or contact their local State Health Insurance Assistance Program to obtain information about all available Medicare options.
The complete CMS-required TPMO disclaimer, including the current number of organizations and products Eterna represents, will be displayed separately on the Website where required.
5. Marketplace Information
Marketplace eligibility, Medicaid eligibility, premium tax credits, cost-sharing reductions, and other financial assistance are determined by the Health Insurance Marketplace or another authorized government agency—not by Eterna Life & Health.
Estimates generated through the Website or an enrollment platform are not final eligibility determinations. Consumers are responsible for reviewing official eligibility notices and promptly submitting any requested documents.
Changes in income, household composition, address, immigration status, tax-filing status, or access to other coverage may affect eligibility and financial assistance. Consumers are responsible for reporting required changes to the Marketplace.
6. Quotes and Plan Comparisons
Quotes and plan comparisons are estimates based on the information available at the time and the information supplied by the consumer.
Final premiums, benefits, provider networks, drug formularies, cost-sharing amounts, subsidy eligibility, and coverage terms must be confirmed through the applicable insurance carrier, Medicare, HealthCare.gov, or authorized enrollment platform.
Before enrolling, consumers should verify:

8. Authorized Representatives and Household Information
If you submit information for another person, including a spouse, dependent, or household member, you represent that you have authorization to provide that information and request assistance on the person’s behalf.
You may not submit another person’s information without permission or impersonate another individual.
9. No Fees for Basic Enrollment Assistance
Eterna may receive commissions from insurance carriers when permitted by law. Consumers are not charged an additional premium because they use a licensed agent.
If Eterna ever offers a separate service that requires a consumer fee, the fee and applicable terms will be clearly disclosed and agreed to before the service is provided.
10. Communications and Consent
When you provide your telephone number or email address and request assistance, you authorize Eterna to respond regarding that request.
If you separately consent to receive telephone calls, text messages, or marketing emails, your consent is not a condition of purchasing or enrolling in insurance. Message and data rates may apply.
You may withdraw consent for promotional communications by:

Withdrawal of marketing consent does not prevent communications necessary to complete a service you requested or address an existing application.
